Welcome to the Grand Valley Animal Hospital family! Our team is happy that you have chosen us to provide your pet with the best possible care. Let’s get started!

We are currently taking new clients and look forward to meeting you and your pet! To establish care with our clinic, we need:

1. Pet's Full Medical History

Please make sure to send your pet’s previous full medical history (if applicable).

2. Deposit of $75

This will cover the cost of your exam. If you need to change or cancel the appointment, we need a 24-hour notice. If a 24-hour notice is not given, the deposit is forfeited.

3. Completed New Patient Form

Please fill out and submit the form below, then call the clinic to pay the deposit and set your appointment. 

Please note: If paying with a credit card, there will be a 3% surcharge.

  • Provide your pet’s complete medical history – including vaccines and doctor’s notes – by uploading all records here, or by requesting your previous veterinarian to email them to us at info@grandvalleyvet.com. Please note: If you do not provide proof of rabies vaccination for your pet/s, we will require that this vaccination be administered by our team at your pet’s exam.

    For your convenience and to help create a less stressful environment for our patients, we offer texting for appointments, pick-ups and more! When you arrive at our clinic, text us to let us know you’re here and our team will be out to help you and your pet.

    Important Payment Process Update

    Due to rising credit cards costs, we will implement a 3% surcharge on credit card payments effective April 1st. Payments made via debit cards or cash will not be affected.
